What to include (17 U.S.C. §512(c))
1) Identification of the copyrighted work. 2) Identification of the material claimed to be infringing (URL). 3) Your contact information. 4) A statement that you have a good-faith belief that the use is not authorised. 5) A statement, under penalty of perjury, that the information is accurate and you are authorised to act. 6) Your physical or electronic signature.

EU notice-and-action (DSA Art. 16)
EU rights holders can use the same address. We will acknowledge receipt and decide without undue delay.
Counter-notice
If you believe content was removed in error, you may submit a counter-notice with your contact details, the material, and a statement under penalty of perjury.
